Tell us what you are trialling and what you already know about the client. The more you can fill in, the closer the chair will be to right on the first visit. Nothing here is compulsory apart from the few marked fields, so send it through even if the assessment is not finished.

A consultant will be in touch to confirm the date, the equipment and what we will bring. We cannot always hold the exact time you ask for, and we will tell you straight away if we cannot.
